Week 1 Discussion Quality Improvement, RCA, and the PDSA Model in Clinical Practice Quality Improvement (QI) teams and Root Cause Analysis (RCA) are essential in promoting patient safety and advancing clinical practice. QI teams systematically evaluate processes and outcomes to identify areas for enhancement, while RCA allows clinicians to investigate the fundamental causes of adverse events, rather than focusing on individual blame. This process supports a culture of transparency and ongoing learning in healthcare settings (Institute for Healthcare Improvement [IHI], 2023). The Plan-Do-Study-Act (PDSA) model aligns closely with evidence-based practice by offering a structured, iterative approach to testing and refining clinical interventions. In this model, teams plan a change based on available evidence, implement the change (do), evaluate the outcomes (study), and make necessary adjustments (act). This cycle supports the translation of evidence into practical, measurable improvements in patient care (Agency for Healthcare Research and Quality [AHRQ], 2020). In my oncology unit, a recent QI initiative focused on reducing central line-associated bloodstream infections (CLABSIs). Through PDSA cycles, we introduced a standardized central line maintenance bundle. Initial observations revealed inconsistent compliance, prompting updates to training protocols and workflow reminders. Over time, adherence improved, and infection rates significantly decreased. While effective, the PDSA model also has limitations. In high-acuity settings like oncology, time constraints and staffing pressures can limit full engagement in all four phases. Additionally, without consistent leadership support and frontline participation, improvement efforts may not be sustained. Despite these challenges, PDSA remains a valuable tool for integrating evidence-based strategies into clinical care.
